NEW PLAYER INFORMATION AND FORMS
New players must complete the below forms prior to registration:
1. Hockey Alberta Parent Declaration - please complete and put office@sgmha.ca in the Forward Submission section. Proof of residency is required. Please see the Proof of Residency requirement form here.

All parents need to have a valid Respect in Sport - Parents Course prior to your registration being accepted. Please log into your Respect in Sport log in to check on the certificate date.
If you do not have one, please complete that now. YOUR REGISTRATION IS NOT COMPLETE IF THE RESPECT IN SPORT - PARENT COURSE IS NOT VALID.

U11HADP Tryout information - SGMHA is proud to host a U11HADP program and will be hosting tryouts around the first weekend in September. Please watch the website for more information. and dates.
RESIDENTS WHO ARE NOT FROM SGMHA INTERESTED IN THE U11HADP PROGRAM, PLEASE REGISTER WITH YOUR HOME ASSOCIATION FIRST. OUR TRYOUT REGISTRATION WILL OPEN MAY 21.
